5) Bank Passbook First Page Xerox or Cancelled Cheque

6) After Selection as Insurance Advisor candidates has to pay IRDA exam fees Rs 500 + LIC Registration Fees Rs 150 + Study Materials Rs 50 + Online Training Certificate Fees Rs 100 = Total Rs 800
