major roles & responsibilities

dispense medications

accurately dispense prescribed medications to inpatients and outpatients.

verify prescriptions

review prescriptions for accuracy, appropriate dosage, and potential drug interactions.

patient counseling

educate patients on proper medication use, side effects, storage, and adherence.

monitor drug therapy

collaborate with doctors and nurses to ensure safe and effective drug therapies.

inventory management

maintain proper stock levels, manage expiry dates, and ensure secure storage of medications.

medication reconciliation

review and update patient medication history during admission, transfer, and discharge.

ensure regulatory compliance

adhere to hospital protocols, pharmacy laws, and regulatory standards (. nabh, fda).

prepare iv & compounded medications

mix iv solutions, chemotherapy drugs, or other special formulations as required.

participate in clinical rounds

provide input on drug choices, interactions, and treatment plans in multidisciplinary team discussions.

pharmacovigilance & adr reporting

monitor and report adverse drug reactions and participate in safety programs.

maintain records & documentation

keep accurate records of drug dispensing, controlled substances, and audit reports.

train & guide staff

assist in training junior pharmacists, interns, or pharmacy assistants.
