Registered NurseVisit patients homes and provide them with a full range of Home Healthcare ServicesLocation: ChennaiJob Description:As a Registered Nurse, you will visit patients homes each day and provide them with a full range of clinical care, which may include such items as instruction on chronic disease management and medication, pain management, wound care, IV infusion, and post- operative instruction. Part of your job is to work with patients and provide the tools they need to ensure their own recovery. Seeing the end results of their recovery is just one of the many rewards of your position.Duties and Responsibilities:Providing case management as described in SOP for all cases involving nursing and therapy servicesMaking the initial evaluation visit as applicable and re-evaluating each patients nursing needs on a regular basisProviding nursing services, treatments and diagnostic procedures requiring specialized skills and assuming full responsibility for each of your patientsParticipating in in-service programs, as well as training, and teaching other nursing personnelDeveloping and implementing the treatment plan for each patient under the direction of their physicianObserving and reporting symptoms, reaction to treatments, drugs, and changes in the patients physical or emotional conditionEvaluates and revises the plan of care, as necessary.Provides those services requiring substantial specialized nursing skill based on the physicians orders and the nursing diagnosis. Examples of such services include, but are not limited to, physical assessment, patient and family teaching, urinary catheter insertion and care, venipuncture and administration of IV fluids or medications, dressing changes, sterile and non-sterile etc.Acts as the coordinator of the health care team in order to maintain the proper linkages with a continuum of care.Provides reports to the patients physician about patient under care when the patients condition changes or

To assess, monitor and evaluate clinical development of patient and reassess care needs from time to time. Identify the level of care required Prepare/approve care plan. Ensure continuum of care given. Ensure medico legal compliance. Home visit for patients. Management of medical emergencies Ensure entry of patients clinical data in EMR Identify abnormalities/deviation in EMR and provide appropriate medical advice. Inform and record consultants advice and follow. To team build allied care givers and co ordinate with allied partners Train in house medical and nursing staff in the class room and on site to provide quality care. Teach and participate in continuous medical education and community health awareness program. Council/ Teach/Train patients to take care of their daily activity and self- medication. Promote our services by participating in brand promotions, camps and community programs. 24-hour on call coverage and any additional responsibilities as required by the management on case to case basis. Report and update all activity to assign authority.
