DESCRIPTION:

Key Responsibilities:

Review patient medical records, clinical documentation, and physician notes.

Assign accurate medical codes for diagnoses, procedures, and treatments.

Apply coding guidelines and standards, including ICD-10-CM, CPT, and HCPCS, as applicable.

Ensure coding accuracy and maintain quality standards.

Identify missing, incomplete, or unclear documentation and seek clarification when required.

Follow healthcare regulations, payer guidelines, and company policies.

Maintain patient confidentiality and data security.

Meet daily productivity and accuracy targets.

Coordinate with the quality assurance team and other healthcare professionals.

Stay updated on changes in medical coding guidelines and regulations.

Required Skills:

Knowledge of medical terminology and human anatomy.

Understanding of disease processes and clinical documentation.

Basic knowledge of ICD-10-CM, CPT, and HCPCS coding systems.

Good analytical and attention-to-detail skills.

Basic computer skills and proficiency in MS Office.

Good communication skills and ability to work in a team.

Ability to meet deadlines and productivity targets.
