Care Companion

* Gather pertinent information about the patient's medical and family history record information in the forms provided.

* Coordinate with the counselors and patients to assist in organizing while closely working with the patient caregivers and doctor's/hospital's team.

*Assist cancer patients/caregivers with accessing all the available resources for cancer patients that are provided by the doctor/hospital.

* Work with the central team to provide counseling support and community program support to patients based on their need.

*Provide emotional support to the patients as needed.

* Be an advocate for the patients.
