Looking For Patient Care Coordinator

1. Gather patient information about the patient's medical and family history record information in the forms provided

2. Coordinate with the counselors and patients to assist in organizing appointments while closely working with patient caregivers and the doctor's/hospital's team

3. Assist cancer patients/caregivers with accessing all the available resources for cancer patients that are provided by the doctor/hospital

4. Work with the central team to provide counseling support and community program support to patients based on their need

5. Provide emotional support to the patients as needed

6. Be an advocate for the patient
