• review patient charts and documents and determine the correct codes.

• ensure proper coding with correct sequence based on the medical records and make sure that all the codes are active for the date of services.

• escalate for missing or incomplete reports and unsigned records.

• determine the codes as per the coding guidelines and ensure compliance with state and federal regulations.

• check the lcd as per the carrier specifications.

• knowledge of handling the pqrs measures.

• follow payer guidelines and identify the reimbursable procedures.

• address all the coding related queries escalated by charge entry and ar team.

• regular feedback to the charge entry team and ar team based on the findings.

• document all the activities in the control log.

• meet productivity and quality standards as set by the department.

• assist with month-end closing activities

• adhere to all company policies and procedures.
