Key Responsibilities:
1. Review and assess patients' medication regimens to ensure safety and efficacy.
- The clinical pharmacist will be responsible for reviewing and analyzing patients' medication orders to prevent medication errors and optimize therapy outcomes.
2. Collaborate with healthcare providers to develop and implement treatment plans.
- Working closely with doctors and other healthcare professionals, the clinical pharmacist will contribute to creating comprehensive treatment plans tailored to individual patients' needs.
3. Provide drug information and counseling to patients.
- The clinical pharmacist will educate patients on their medications, including dosage, side effects, and potential interactions, to promote understanding and adherence to treatment.
4. Monitor patients' progress and adjust therapy as needed.
- Regularly assessing patients' responses to medications, the clinical pharmacist will make recommendations for dosage adjustments or changes in therapy to ensure optimal outcomes.
Required Skills and Expectations:
1. Strong knowledge of pharmacology and therapeutics.
2. Excellent communication and interpersonal skills.
3. Ability to work collaboratively in a team-based healthcare environment.
4. Attention to detail and critical thinking skills.
5. Current pharmacy license in Singapore.
6. Previous experience in a clinical pharmacy setting is preferred but not required.
