medical record review & coding
review patient medical records, operative reports, and clinical documentation to assign accurate icd-10-cm, cpt, and hcpcs codes.
ensure coding accuracy for diagnoses, procedures, and services in compliance with payer and regulatory guidelines.
quality assurance & accuracy audits
perform internal coding audits to identify errors and ensure data integrity.
provide feedback and recommendations to improve coding accuracy and compliance.
compliance & regulatory adherence
ensure all coding practices comply with hipaa, oig, and cms regulations.
stay updated on changes in coding guidelines, payer policies, and government regulations.
revenue cycle support
collaborate with billing teams to resolve coding-related claim denials and rejections.
support accurate charge capture to optimize reimbursement and minimize revenue loss.
clinical documentation improvement (cdi)
work with physicians and clinical staff to clarify incomplete or ambiguous documentation.
educate providers on documentation standards to support correct coding and compliance.
mentorship & team support
train and mentor junior coders, providing guidance on complex cases and new regulations.
assist in performance evaluations and skill development of the coding team.
audit preparation & reporting
prepare and present audit reports and key performance metrics (accuracy rate, productivity, denial trends).
support external and internal audit processes as needed.
specialty coding expertise
handle complex cases in specific specialties (., inpatient, outpatient, surgery, ed, cardiology, or orthopedics).
serve as the subject matter expert (sme) for assigned departments.
data integrity & confidentiality
maintain patient confidentiality and protect sensitive health information in accordance with hipaa.
ensure the integrity and accuracy of coded data for analytics and reporting.
continuous learning & process improvement
participate in coding education sessions, workshops, and credential maintenance (., cpc, ccs, or coc).
contribute to process improvement initiatives for greater efficiency and compliance.
