Job Description

Review patient charts and documents and determine the correct codes.

� Ensure proper coding with correct sequence based on the medical records and make sure that all the codes are active for the Date of services.

� Escalate for missing or incomplete reports and unsigned records.

� Determine the codes as per the coding guidelines and ensure compliance with state and federal regulations.
